You do not need a referral to access this service. Service requests can be submitted by any person with permission to act on behalf of the participant, including primary caregivers and support coordinators.

Following a request for service, our OT will contact you with details including anticipated service costs and timeframes. There is no obligation to proceed with any services based on submitting a request. Follow the link below to submit your enquiry.
